I think the coaching situation was the biggest issue, no one wanted to work with him and he constantly recruited people with no experience of the game, who then didn't last very long. IMO over time, his style of management gets on everyone's nerves - coaches and players, kind of like Mourinho.
Timing wise, i think he benefitted from a decent base built by Lancaster and then picked some rogue grunt to build on top. It started to fall apart after 2019, he wouldn't trust new players and let them learn international rugby in the shirt. Hence we're stuck with Daly / May / Cole et al who probably weren't good enough at their peak and definitely aren't now (probably harsh on May at his peak)
