He called “that’s once” after a couple of pushes backwards and then after several pushes back, he allows play on.
Mauls (if we can still call them that) suck.
Ref had made it perfectly clear he was going to allow Ireland to get away with everything on the line there whilst penalising the MAB for everything.
It was always going to be a yellow for MAB & 7 points for Ireland from the second they got there.

I really hate they are not picking up the offsides plus the cleanouts way beyond the ball

To be fair, there's a lot of offside by MAB on defence not being picked up.

Yes, you can pass a ball to an Irishman directly behind the ruck because that's a perfectly onside position.

Open play I thought. Perenara popped it up to no one, he came through from his side and caught it. Looked fine
