The framework didn't differentiate between contact initiated by the defender or the attacker. Shoulder to head is very different to head to shoulder, but interpreted the same. I think that's the frustration.

A pathetic decision by the ref. Vunipola leading with his head down isn't a mitigating factor? I'm not sure what Aki is meant to do here.
Good Lord. Just watched a video of it, cannot believe a red for that. Aki dropped, knee was nearly on ground. Unless front on tackles are penalized, that happens multiple times per game.
